Eligibility

1. This giveaway is organized and governed solely by FYidoctors. No other organizations are affiliated with this giveaway. This promotion is in no way sponsored, endorsed or administered by, or associated with, Kate Spade, Hugo Boss or any other brands, organizations or affiliations.

2. Entry submissions are accepted starting at 8:00am (MST) on November 4, 2022 and closing at 5:00pm (MST) November 5, 2022.

3. To enter, participants must fill out the form with their name, email address, and phone number via QR code displayed at the Mission Square clinic.

5. Participants may submit one entry per email address.

6. This contest is open to all legal residents of Canada who are 18 years of age or older. Employees of FYidoctors and affiliated partners are ineligible to enter. Immediate family members of FYidoctors employees and affiliated partners are also ineligible to enter.

Contest Prize

7. Multiple prizes are being offered and prizes are limited to one per winner. Prizing: One (1) Kate Spade Tote Bag. One (1) Hugo Boss Tote Bag. The prize is non-transferable and additional terms and conditions may also apply.

Conduct of This Contest

8. The winner will be determined by 3:00pm MST on November 10, 2022. Winners are also subject to FYidoctors' discretion.

9. Winner will be notified via email. Winner will be required to answer a skill-testing question (e.g., (2 × 4) + (10 × 3) (Answer: 38)) to be confirmed as a winner.

10. The winner’s name(s) and likeness may be used in future FYidoctors marketing and promotional materials (with written and/or verbal consent from the winner).

11. If the prize is not claimed 1 month after the winner has been notified, a new winner will be drawn at random (again, based on FYidoctors’ discretion).

12. The prize is scheduled for disbursement in December 2022.

General

13. FYidoctors may, at any time at its sole discretion and without prior notice, vary, modify, delete or add to these Terms and Conditions and may also withdraw or discontinue this contest at any time without notice or liability to any party. These Terms and Conditions are governed by Canadian federal and provincial law.

14. By entering this giveaway, participants are providing implied consent to receiving marketing emails from FYidoctors and reserve the right to withdraw from or unsubscribe from them at any time.
